Fibromyalgia – Further Elucidation of Small Fiber Neuropathy Presence.

With Both neuropathic level pains, and paresthesias common in Fibromyalgia; the idea Fibromyalgia is a small fiber neuropathy, was presented in 2010. Now an extremely thorough followup was presented again confirming this issue. They found a a “pathological process involving C-fibres” – abnormal function and fiber loss. They felt this affected skin tissues and joints as well. They concluded that  a proportion of FM was a neuropathy and, in their case subjects, not driven by depression.

Antidepressants Just Don’t Work on Descending Inhibition

  • lesion of central descending noradrenergic pathways does not block painkilling effects of Nortriptyline – It was always thought descending “endorphin-like” response was its effect but that should not happen with damage to DNIC
  • the fact damage to peripheral sympathetic NS blocks nortriptyline response suggests its effects as more β2-adrenoceptors related.  It is though this effect on the nervous system blocks cytokine TNF production.
  • It is thought to be a noradrenergic effect so drugs like venlafaxine and duoxetine should be useful. Interestingly, oral asthma drugs like terbutaline (bricanyl) could also be helpful.
  • Comment – will the future involve taking nortriptyline and terbutaline combined to block TNF production?

Dr. Serge Marchand of Sherbrooke when asked about this study, stated there lesion were not accurate so results were not accurate. It seems that another group has taken a crack at knocking out the DNIC circuit to see if amitriptyline will still work:

  • suppression of noradrenergic descending inhibitory systems by chemically knocking out noradrenergic neurons in the locus coeruleus (LC) and spinal cord.
  • amitriptyline [still] produced antihyperalgesic effects against neuropathic pain despite suppression of noradrenergic descending inhibitory systems. Amitriptyline activated LC [locus coeruleus] neurons and increased noradrenergic fibers density in L5 spinal nerve ligation (SNL) rats.
  • this analgesic effect was not reversed by the α2-adrenoceptor antagonist so put in doubt first article’s idea its related to α2-adrenoceptors
  • does find that serotonin and noradrenaline reuptake inhibitors and gabapentinoids require normal descending noradrenergic inhibition.

Comment: – does look like amitriptyline will augment SNRI and gabapentinoids as they have differing effects

1/10th of painful uterine scars will rupture; none in the non painful group. However all rupturing cases had other features – abnormal fetal monitor (4), peritoneal signs (2), high fetal station (2) or vomiting(1).

Chronic Diarrhea? -Think Bile Malabsorption(BAM)

“The estimated prevalence of BAM is >90% in patients with resected Crohn disease (CD) and 11% to 52% of unresected CD patients (type 1); 33% in diarrhea-predominant irritable bowel syndrome (type 2); and is a frequent finding postcholecystectomy or postvagotomy (type 3).” Worth thinking about.

Fibromyalgia(FM) – Quetiapine XR (Seroquel) Reduces Pain Similar to Others and Reduces Depression – But Drowsiness High

FM could be considered a neurodegenerate disorder with certain degrees of brain loss and cognitive impairment evident. Some years ago I was at a presentation in which the brain damage evident in Fibromyalgia was bemoaned by speaker. Having dealt with arresting that in bipolar patients with mood stabilizers, , I asked the speaker after if he though about using neuroprotective agents.  He had never thought about it. Quetiapine may help nerve repair. Quetiapine helps pain and depression in a recent study. Could it quell the brain deterioration as well?

Would Small Amounts of Lithium Help the Brain- Maybe

In Lancet 2000, it was found that there could be a 3% increase in grey matter with 1 month of of lithium therapy. Sadly, it is not that easy. Some say it would take 300+ mg/day to do anything though one study suggests Alzheimer’s protection at 0.3 mg(300 micrograms)/day.

Medial Femoral Condyle Knee OA – Is a Medial Plica the Cause?

Removal of the medial plica in cases of symptomatic plica with femoral condyle OA resulted in 88% excellent results at 6 months and 94% excellent results at 1 year. Conclusion was the plica was helping precipitate the OA.

Geriatric Rotator Cuff Tears – Intra-articular Shoulder Joint Steroid Injection For Pain Control

Recently, had an elderly but otherwise in good condition patient with tears of two of his shoulder rotator cuff tendons that he wished repaired. Was told by specialist – too far gone – and just learn to live with it. Now it looks like intra-articular steroid injection can almost cut pain in half for 3-4 months. A repeat shot at 3 weeks had no additional benefit so maybe it should be repeated at 3-4 months?

Increased Connection Of Medial Prefrontal Cortex to Nucleus Accumbens(pleasure center) Predicts Chronic Pain

Saw recent talk by Dr. Apkarian  where he noted overactivity of nucleus accumbens (pleasure center) in chronic pain – he called it “addicted to pain” (no relation to painkillers). Now this appears to be driven by increased white matter circuitry from medial prefrontal cortex (a thought center) with nucleus accumbens . This increased circuitry “robustly” predicts transition to chronic pain. They speculated that the “brain structural differences, most likely existing before the back pain-inciting event and independent of the back pain, [and] predispose subjects to pain chronification”

Effect Of Cushioning Insoles on Workplace Back, Foot and Knee Pains

Use of sorbothane insoles cut  back pains by 38%, foot pains by 37% and knee pains by 38%. Relief in back pain did not mean relief in another areas. Wonder if antifatigue covers would help more.
