Efficacy of sodium hyaluronate in the management of chemical and radiation cystitis.

Cystistat, available in Canada but not the US, is used in In IC:
see ad here
It is often given weekly and costs $280.00 US dollars per weekly 50 ml vial. One supplier here
In IC, it works best after a bladder distention procedure.
There is a protocol for use for radiation cystitis,or after chemotherapy or BCG instillation:
- sodium hyaluronate, 40 mg/50 mL, weekly for from 8 to 24 weeks
- first four weeks dexamethasone 32 mg was mixed in as a “cocktail”,
- “to allow patients with marked overactive bladder to keep these drugs within the bladder, we instilled lidocaine 2% 30 mL, 30 minutes before” – this is a treatment in itself – it has been found to be very beneficial in Interstitial cystitis:
Intra-Bladder Lidocaine Freezing to Diagnose Bladder Pain Syndromes and Lidocaine Treatments for Bladder Pain
30 mls of 2% is 600 mg of lidocaine – I’m not sure I’d want to use more than 10 mls but I guess they do pee most out after 30 min… - though pain and frequency disappeared after 4 weeks, it was necessary to keep up treatments as great tendency to relapse
- bladder capacity went from 85 to 243.3 mL (P<0.0001) in the radiotherapy patients
“Overall 67 patients (97%) reported complete relief of dysuria and pain”
comment – looks good but an 8 week treatment would be $2000 dollars for just cystistat alone.
